Release and Consent

I hereby acknowledge and give my permission to participate in the physical activities and programs indicated above, delivered by Peter Hollowood Martial Arts (The Dojo of Champions).

I agree to release and indemnify the facility operator and staff as follows:

1. The information stated is to the best of my knowledge correct and I will inform you of any changes.
2. I understand the benefits of physical activity outweigh the risks, but in a small number of people injuries may occur.
3. I acknowledge that whilst on School’s premises or any other place where I attend training and during all training times, both my property and my person shall be my own risk and I will not hold the school or its members liable for any property loss and damage or personal injury.
4. I understand that in the case of an emergency I will be treated as per the current policy of Peter Hollowood Martial Arts (The Dojo of Champions).
5. I understand that the school reserves the right to terminate my membership at any time, following which I will only be liable for any outstanding training fees up to the date of membership termination.
6. I participate in the activities at my own risk and sole responsibility; I release, indemnify and hold harness the facility operator, its servants and agents, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever, I also agree that in the event of injury or if property is lost or damaged, I will bring no claim, legal or otherwise against the facility operator, its servants or agents, in respect of that injury, loss or damage.
7. I give permission for photo & video footage of myself (or the student hereby represented) training and / or competing to be used for promotional and advertising purposes.
8. I understand that the karate school may be closed during the Christmas period (no longer than 2 weeks), this time is included in the training calendar and fortnightly fees will apply.

Conditions

There are no refunds on the once off joining fee.
